Automated, personalized voice bot calls directly to the patient’s phone. With personalized and targeted questionnaires, Eleanor engages with patients over the phone to inquire regarding their health and functional status and elevates highlighted transcripts with crucial health-related information to the care management teams when necessary. Eleanor can be activated upon risk-detection from the MyndYou App (when smartphones are applicable), or on a routine basis, and transcripts following calls with Eleanor will be elevated to the care management teams through the MyndYou Care Portal.
